Personalized Care

Attention Plus Care staff can provide a full range of home health care and companion services including: light housekeeping, meal preparation, grooming, bathing, dressing, exercising, companionship, administration and assistance with medications and treatments, and much more. Our care is always personalized and can always be adjusted to higher or lower levels of care depending on our client’s needs.

Flexible Services

Our home care services are available 7 days a week, 24 hours a day, with prescheduled programs. And our flexible services have made it a very successful home care program. Whether the need calls for one hour per week or 24 hour coverage, we provide a flexible alternative that adds value to our client’s home care choices. Care can be arranged to meet routine or complex care needs in the privacy and comfort of the home, or also at other locations such as a hospital or hotel.

Care Services Include Free or Without Additional Charges:

  • Free initial consultation with our Service Consultant.
  • Initial assessment by Registered Nurse Client Service Supervisor, including home and environmental safety assessment.
  • Regular visits and telephone calls by Client Service Supervisor to assess client needs, oversee progress, and supervise employees.
  • Registered Nurses on-call 24 hours a day, 7 days a week for urgent telephone consultation.
  • Individualized plan of care and maintenance of care records.
  • Ongoing caregiver scheduling support.
  • Carefully screened and selected employees based on skills, attitude, client compatibility, and sincere interest in delivering quality care.
  • Employees covered by workers compensation, temporary disability, bonding and professional liability insurance.

Our Care Plan

Care can be arranged to meet routine or complex care needs in the privacy and comfort of the home, or also at other locations such as hospitals, assisted living residences, or hotels. Our professional staff develops and maintains an individualized ‘care plan’ for each client that includes agreed upon goals and a schedule of care to be provided. And because we know that care plans and conditions may change from time-to-time, our clients can access their caregiver schedules online anytime of the day or night.

Care Levels

We understand home care needs differ from person to person, that is why we created four levels of care. Each Care Level is more advanced and accommodating than the previous and is always provided by Professional Caregivers and Licensed Nurses. We have additional services to assist you through our Care Management and Specialty Services.

Worldwide Travel Assistance

Attention Plus Care provides reliable and skilled Worldwide Travel Assistance to individuals in need of nursing care while in transit to domestic and international destinations worldwide. Our professional team of licensed nurse employees often can respond to even the most urgent request for travel assistance.

Live-In Care

Live-In Care is an arrangement that provides continuous, supervised home health care services in a manner that maintains the client's independence, security, health and safety, all at a reasonable cost.

The primary caregiver in this arrangement is the live-in employee of Attention Plus Care who moves into the client's home and is provided with room and board. The Live-In works as needed with the client throughout the week on a 24-hour basis. However, the Live-In must have at least 12 hours of personal time off per 24 hours for such things as sleeping, eating, and bathing. The live-in caregiver will stay on premises during these periods of personal time off.

Live-In employees require 35 additional hours off per week during which they can leave the premises when they are completely relieved of care giving responsibilities. This relief time promotes caregiver well being and avoids "burnout." Attention Plus Care offers two options for scheduling relief. In the "Scheduled Relief" option, Attention Plus Care takes responsibility for scheduling respite caregivers for the Live-In.

All such respite caregivers are employees of Attention Plus Care, and they are responsible for attending to the client's needs while the Live-In is off-duty. A second option is called "Client/Family Relief." Here the client or the family arranges for relief of the Live-In. Selecting this option results in lower weekly costs of services for Live-In coverage.

Multiple Person Care

When two or more people in the same household need our attention, we offer Multiple Person Care. For a special economical rate our companions, health aides, or licensed nurses can attend to your needs as well as the needs of your husband, wife, child, or other family member who resides in your home.

Care Management

Some clients and family members may need help with planning, coordinating, and navigating through today’s complex healthcare systems, and in these cases our popular Care Management Consultation services can be of great value and benefit.

Care Management Consultation is a cost-effective way of obtaining guidance and expertise from experienced licensed nurses to help manage care provided at home. Common examples in Care Management include: arranging for family respite, help with client care planning, hospital discharge planning, case management assistance, complex care coordination, client advocacy, inter-office health care coordination, health records keeping and documentation, help with insurance claims, making assessments, client education, caregiver skills assessment and training, and much more.

Our Care Managers are Licensed Registered Nurses with training and experience in home health care to handle your care needs, and take the stress out of your day-to-day life.

Working with health care and other service providers, our Care Managers will develop a care plan that is right for you. They can manage your appointments and keep you informed all along the way, so you can spend time doing other things that are important to you.

Education and Training

Becoming a caregiver to a person you love can be a rewarding experience. At Attention Plus Care, we offer specialized training to help you learn about health care and treatment, along with proper safety techniques.

Learn How to be a Caregiver:

  • Helping with basic life tasks, such as bathing, dressing, toileting, and eating.
  • Monitoring bowel movements for signs of constipation or diarrhea.
  • Dealing with proper wound care.
  • Learning how to care for someone suffering from dementia or Alzheimer's disease.

Attention Plus Care understands this, so we have developed a care package tailored to suit these needs. Care Management is designed to look after health care needs as well as personal needs. Let us deal with the everyday nuances, so that you can have more free time to enjoy what's important in life.

Our Care Managers can help by:

  • Assessing your physical and mental status, environmental and home safety, and needs for temporary or continued care.
  • Developing a Care Plan that will address all care needs, set goals for healing, and include instructions for caregivers to follow; as often as needed, we'll re-evaluate the plan, and make revisions.
  • Collaborating among family members and providers such as physicians, social workers, pharmacists, and hospital discharge planners.
  • Ensuring that proper medical follow-up is being done to meet specific care needs, such as scheduling and attending regular doctors' appointments and diagnostic tests.
  • Helping you to manage different medications.
  • Arranging short term and long term stay at rehabilitation facilities, assisted living homes, or nursing homes.
  • Helping to find physicians and other health care providers.
  • Researching and explaining insurance coverage.
  • Seeking local resources for home medical equipment, meal delivery, grocery service, medications, and transportation needs .
  • Helping with personal needs such as finding leisure activities, making hair appointments, locating companies to help with home repairs, pest and insect control, and public utilities.
  • Setting up a web based health information management system so that you, your family, and caregivers can easily keep track of your appointments and medical needs, and monitor progress or changes that occur.
  • Training you, your family, and caregivers how to use the on-line service, and how to enter important information such as appointments.

Medication Management

Having to keep up with various medications and medication schedules can really be confusing. With multiple doctors and hospital visits things can get overwhelming. We have a proven system to help you or your caregivers keep track of different medicines, make sure you get the medications you need, determine what times they are to be taken, and when they need to be refilled.

Our Care Managers can help by:

  • Obtaining orders from physicians to verify all medications.
  • Pouring and sorting the exact medication doses needed into pillboxes according to the right time of day and day of the week.
  • Updating the previously sorted medications when the doctor changes dosage or drug type.
  • Reviewing special instructions on how medication should be taken and different drug interactions, so that you or your loved one can be kept from harm.
  • Calling the pharmacy when refills are needed and calling physicians when new orders need to be placed.
  • Educating family members and caregivers on how drugs should be taken, the purposes of your medications, and any side effects that may occur.
  • Picking up medications, or coordinating delivery by mail, and purchasing other items needed for safe care.
  • Monitoring to ensure that medications are being taken properly, and that the medication is working, and informing physicians of any adverse effects that drugs may be causing.

Emergency Preparedness

Before disaster strikes, Attention Plus Care is here to help you prepare. We have developed many specific plans for whatever nature might bring upon us. Whether it be a hurricane or a house fire, we help make sure that your medical needs are taken care of, and that you have the supplies to make it through the storm.

Our Care Managers can help by:

  • Conducting an environmental safety assessment in your home.
  • Obtaining home emergency and evacuation supplies.
  • Developing specific emergency plans based on the type of disaster:
    • Hurricanes
    • Tsunami
    • Flooding
    • Fire
    • Power Outages
    • Pandemic Disease
  • Advising you about emergency supplies so that you are ready when disaster strikes, and assisting you in purchasing supplies as needed.
  • Training your caregivers on how to react during an emergency.
  • Providing directions to the nearest emergency relief shelter, and instructions about what to bring with you.
  • Making sure that you have transportation to the relief shelter, and that you and your caregiver know who to notify if evacuation is needed.
  • Educating your loved ones about your emergency plans.
  • Helping you find an emergency response caregiver if this is appropriate.

Advocacy

Communicating among insurance providers, hospitals, doctors offices, medical suppliers, and family members is a full time job. We are here to help. Attention Plus Care can be a voice for you in your time of need. We will speak on your behalf, ask the right questions, relay information to you and your loved ones, even accompany you to doctor visits.

Our Care Managers can help by:

  • Visiting with you while you are in the hospital or other health care facility.
  • Ensuring that your care needs are met by speaking to health care facility staff.
  • Helping with discharge from a health care facility to home.
  • Deciding if home care service is needed and what level of care is appropriate.
  • Obtaining medical supplies before discharge from the hospital, so that you have everything you need waiting for you at home.
  • Meeting with your health care team members to create and maintain a Plan of Care.
  • Reviewing your treatment plans such as surgery, radiation therapy, and chemotherapy.
  • Answering all questions you and your family may have about your diagnosis and treatment plan.
  • Finding doctors that can provide second opinions.
  • Keeping a current list of questions and concerns that you have regarding your care and condition.
  • Accompanying you on your doctors appointments and acting as a spokesperson.
  • Keeping local and long distance family members and caregivers informed about your overall health and progress.
